There are no centralised standards
Failures data entry testing.
Deposit amount verification and data duplication problems.
UK Inland Revenue tax return is too complex.
Very familiar but well designed?
is something that the majority of us see as a necessary evil.

We begrudgingly fill in our annual tax returns, apply for loans, and sign away vast sums of money on the dotted line. We try to write in tiny boxes that require the precision of a fine artist to maintain legibility. We wade through paragraphs and pages of fine print written in a terminology intent on obscuring the message. We answer questions with only a vague understanding of what we are doing.

It is time to question form design - it is just bad form to have bad forms.
